NAD+ Therapy and Anti-Aging: What We Know, What We Don't, and Honest Expectations
NAD+ (nicotinamide adenine dinucleotide) is a coenzyme present in every cell, central to energy metabolism and to several pathways involved in DNA repair and cellular stress response. Cellular NAD+ levels decline with age. From those facts, the wellness industry has built a large story. Some of that story has reasonable mechanistic support. Some of it does not.
What current evidence supports
There is reasonable laboratory and animal data showing that boosting NAD+ levels improves measures of mitochondrial function and certain markers of cellular stress. There are small human studies suggesting subjective improvements in energy or mood for some patients. The evidence base for systemic anti-aging effects, lifespan extension, or treatment of specific diseases is much thinner and frequently overstated in marketing materials.
What patients typically experience
NAD+ infusions are generally given slowly, often over two to four hours, because rapid infusion produces significant chest tightness, abdominal cramping, and a sensation many patients describe as unpleasant. Most people who tolerate the session well report a few days of improved energy and mental clarity. Whether that is the molecule or the rest is honestly unclear from a clinical standpoint.
Who should not consider NAD+
Patients with active cancer should not pursue NAD+ infusion outside of an oncologist's specific guidance, because the same pathways NAD+ supports in healthy cells are exploited by some tumors. People with certain cardiac arrhythmias, severe kidney disease, or active infections should also defer. A physician-led screening conversation is non-negotiable for this particular therapy.
Honest framing
If you go into NAD+ infusion expecting to feel sharper for a few days and treating it as one tool among many (sleep, training, nutrition, stress management) the experience is usually positive. If you go in expecting it to reverse aging, you will be disappointed and will have spent a lot of money. We discuss this directly with every patient who asks about it.
